90785: Interactive complexity add-on
90785 is an add-on that flags interactive complexity: factors that make communication harder and change how you deliver the session. It rides on 90791 and on the timed psychotherapy and family codes; it never stands alone.
When to use it
Add it when at least one applies and you document it: play equipment or other physical devices needed to communicate with a young child; a third party such as an interpreter or a translator; managing maladaptive communication among participants (for example a caregiver who repeatedly disrupts); or a mandated report or disclosure of a sentinel event that has to be discussed with a caregiver.
When not to
Do not add it just because the client is a child, because a parent was present, or because the session was hard. Do not add it to 90846 (patient absent) or to E/M codes.
With children and families
Young children in play-based sessions on video often meet the play-equipment criterion, but only if you record what was used and why. A parent who talks over the child is not by itself maladaptive communication; document what you had to manage. Note the specific factor in the note.
