90837 is the long individual session: 53 minutes or more of face-to-face psychotherapy. It carries the highest scrutiny of the three timed codes because payers watch for routine 60-minute billing without a clinical reason.

When to use it
Use it when the therapeutic time is 53 minutes or more and the plan calls for it: trauma-focused work, EMDR, family-inclusive sessions with the child present, or teens with complex presentations. Say why the longer session was needed.
When not to
Do not bill 90837 for a 50-minute session (that is 90834). Do not make it the default for every client; payers audit patterns. Do not count documentation, scheduling, or parent messaging outside the session.
With children and families
Sixty minutes with a child is a lot of session. It fits when the child and parent are both present for extended work, for exposure work, or for older teens. Document the clinical reason for the length and the minutes with each participant. If most of the hour is with the parent alone, the code is 90846 or 90847, not 90837.
